Registration Requirements

ü     Existing member re-registration: Each of the following items is required for each participant you are registering. 

     They need to be brought to the re-registration date, along with the registration fee**.

 We also need a copy of the participants birth certificate.

Membership Fees

·         Membership Fee is $150 + $25 Community Membership Fee for a total of $175 Per athlete

·         The Community Membership fee is $25 and is mandatory. This fee goes towards maintenance and upkeep

        of the field and associated buildings; it will also gain you a one year membership to the community center.

        This fee can be waived if the athlete is already a Community Center Member.

        A copy of the card must accompany this registration form.

·         These fees are non-refundable.  Make sure of the commitment before paying your fees.

·         Optional fundraising may be used to earn this fee. 

        Fundraising forms can be picked up on January 26th from 1:00-3:00pm.

·         Payment is due by 4/30/08.  Balances not paid by this date will be assessed a $50 late fee.

·         No equipment will be issued until membership fee is paid.

·         The last date to make payment and still retain membership is 6/30/08.

Volunteer Commitment:

During the 2008 season there is a 2 shift commitment per participant commitment.

Sign up will be done after the 2008 schedule is released

ü       Each participant must have physical form on file prior to August 1, 2008 in order to practice. 

    The physical form can be the one provided or an alternate.  It must be signed by a doctor, physicians assistant or

     nurse practitioner and include the doctors stamp and date of physical.

ü        The following information is the current league requirements for player and cheerleader participation.

    It is club policy to assign players to the lowest possible level.  For example, an 11 yr. old will be assigned to

    the B-Team unless their weight is over 125lbs.  Cheerleader squad assignments are more subjective and

    will consider age, experience and the number of participants.

If full payment of membership fees or fundraising money is not received by April 30, 2008,
a $50 late fee will added to your amount due.  

NO EXCEPTIONS.

All fees must be paid in full by June 30, 2008or your child's registration will be denied.
